If you have not already submitted your initial application for licensure or registration, your Live Scan results may not automatically attach to your application. We recommend submitting your initial applicationfirst, and then completing your Live Scan fingerprinting.

Please note that these Live Scan forms will only work for your Optometry application or your Optician application. These forms will not provide results to any other agency or office.
